    JOB PURPOSE SUMMARY: To provide quick, efficient, and proficient
    means for the management and release of protected health information (PHI) to
    multiple healthcare facilities. The disability form specialist is a direct contact to both the patient and the medical facility and provided the necessary documentation in order for the patient to be relieved from work on both Short & Long Term basis. 


    MAIN JOB TASKS AND RESPONSIBILITIES

    •  Understand and abide by all HIPAA privacy and security laws.
    •  Interpret and apply HIPAA rules to release of protected health information requests.
    •  Validate all incoming requests for PHI.
    •  Verify patient information using key identifiers.
    •  Conduct quality screenings on incoming PHI to protect patient data.
    •  Verify requesting party contact information including fax number or address.
    •  Update and maintain an Accounting of Disclosures Log for all medical records released.
    •  Provide attention and care to patients and patient representatives.
    • Ensure patient's disability forms are completed after payment within 2 business day turnaround
    • Pull patient's forms on a daily basis for invoicing 
    • Check voicemails and return calls on a daily basis

    willing to expend in return.  You will be required to review, evaluate, and process Disability/FMLA forms to include the completion of Short Term, Long Term and Family and Medical Leave Paperwork for patients that are seeking short term leave from work.
